Confidentiality & Liability Waiver

Confidentiality Agreement

All information received by the coach from the client shall be held in strict confidence by the coach and anyone working for ThreeOwl Awareness.  This includes any and all information received verbally, electronically, in writing, or any other form of communication.  All such information shall remain in confidence after the counselling/coaching relationship has ended.

By entering into this Agreement, the Client recognizes that:

1. Life Coaching is an unregulated medium, and the Coach has not yet been licensed or approved by any related professional governing bodies.

2. The Coach is not qualified to diagnose or treat physical or psychological medical conditions. 

3. It is the Client’s responsibility to ensure the Client’s fitness to receive counseling and/or coaching services, and to seek treatment, including diagnosis, from qualified medical professionals when required. 

4. Waives and releases from liability ThreeOwl Awareness, and its principal John Onorato, from any and all claims for damages or loss that may arise directly or indirectly from the counseling and/or coaching services delivered to the Client. This waiver extends to claims in contract, tort or otherwise, including (but not limited to) claims of negligence, or physical and psychological injury. 

5. Agrees that this waiver may be in addition to any other consideration that passes between the Client and the Coach, and, where signed in addition to any other contractual documents, forms a severable and enforceable part, addition or addendum to those documents.
